SERVICES AVAILABLE UNDER THE CONTRACT

In addition to the core services available under the contract, eg chronic disease management, terminally ill, general patient care, all investigations and appropriate referrals, we also undertake to offer any direct, enhanced or local services eg Warfarin monitoring, flu vaccinations etc, in accordance with contracts established with Lincolnshire Primary Care Trust.
