HOW TO REGISTER AS A PATIENT

If you move into our practice area, then you are welcome to register with either of our
surgeries. It is important for all new patients wanting to register with the practice to complete
all the necessary paperwork and attend an appointment with one of our practice nurses for a
health screening examination.

All patients are registered with Glenside Country Practice and not with a specific doctor. When
you register with us you may wish to name a preferred doctor and this will be recorded in your
notes. Whenever possible we will endeavour to ensure that you will see your preferred doctor,
however, this may mean that you have to wait longer to be seen or be asked to attend either
one of the surgeries.
